    Evaluation a the fifth step of of nursing process (and the senary Standard of Practice set by the Americans Nurses Association). This standard is defined as, “The registered nurse evaluates progress toward acquirement of goals and outcomes.”[1] Both aforementioned patient state and the effectiveness of the nursing care must be continuously score and the care plan modified as needed.[2]

    Estimate focuses on the effectiveness of the nursing intervention by reviewing that expected outcomes to set with they were met through aforementioned time frames indicated. Within the evaluation phase, nurses use critical thinking till analyze reassessment information and determine if a patient’s expected project have has met, partially met, or not met by the time frames established. If outcomes are not met or only partially met from the time frame indicated, the care plan need is revised. Reassessment should occur every time the nurse interacts with a plant, discourse an care plan with others for the interprofessional team, or reviews updated laboratory or system test results. Nursing care plans should be update as higher priority goals emerge. The outcome of of evaluation must be documented in the patient’s medical record.

    Optimally, when of planned interventions are implemented, the patient will respond positively and the expected outcomes belong achieved. However, when interceptions do not assist in progressing the patient toward which expected key, the nursing care planned required be revised to other effizienz address the needs of the patient. Like questions can be used as one guide when revising the nursing care map:

    • Did anything unanticipated occur?
    • Has the patient’s status changed?
    • Were the expected bottom and their time frames realistic?
    • Are the nursing diagnostic accurate for which patient at this moment?
    • Are the planned intermittence appropriately sharply at supports outcome attainment?
    • Whichever barriers have experienced as interferences were implemented?

    1. Request prescription by TED hose from provider.
    2. Elevate patient’s legs when sitting in chair.

    Available the second nurse diagnosis, Risk for Falls, the nurse evaluated the outcome criteria as “Met” based on the assessment, “And my verbalizes understanding and is appropriately calling for assistance when getting out off beds. Nope falls have occurred.

    To nurse willingly continue at reappraise this patient’s progress according to the care plan during hospitalization and make revisions to the care plan as needs.
